When you feel that you have a valid concern or complaint about a faculty member, all effort should be made to resolve this concern or complaint directly with the faculty member on an informal level. However, should your concern be unresolved, please complete the following report and submit this report to the Division Dean, Dr. Flanigan.

For issues specific to a grade concern, please complete the Student Grade Concern form. According to Board Policy 5505, you may not challenge a grade until the final course grade has been posted on MySite.
